What is it?
You may, or may not be aware that once you are over 75 you are eligible for a free annual health assessment. This is part of the Federal Government’s initiatives to improve the health care available to older Australians. The Health Assessment is not compulsory but we believe that it may help us identify any health needs you may have. An early intervention will allow you to enjoy better health for longer.
All Australians over 75 are eligible for a funded Health Assessment through Medicare. We invite all newly over 75’s to a health assessment, and will continue to send an invite once a year to all over 75’s (unless we have been asked not to).
There is no cost as it is Bulk Billed through Medicare, this includes a nurse home visit (or practice visit if the home is not viable) and a follow up visit with your GP to finalise your assessment.
What happens in a Health Assessment?
The initial stages of the Health Assessment is in your home with a practice nurse. Then completion of the Health Assessment is carried out by your usual GP at the practice. We will try to book this visit within a month of your home visit.
The Health Assessment is a really detailed look at such things as your general health, including sight, hearing, diet, sleep and any medication you might be taking. It also includes any help you may be receiving at home (e.g. home help, meals on wheels) your immunisation status; and, if you have any problems with shopping, dressing or bathing etc. In a home visit we can see if there are any services, equipment or care that you could be using to help with your quality of life.
I have been invited to one or would like one, what do I do now?
This is the easy part, just call us to book for an over 75 year old health assessment and we will take care of the rest.
We will book a time that is suitable for our nurses to come to your home, we sadly cant get everywhere, so sometimes we may need to book you in the practice instead. When we book a home visit please let us know if there is anything we should be aware of that could get in the way of us getting to you; like a difficult to find home or front door, or a big cuddly but slightly scary looking dog!
