Dear Patients,

The My Health Record “Opt out” has been extended to the 31st January 2019. To find out more on how to opt out, and for more information on the My Health Record please follow the following link https://www.myhealthrecord.gov.au

The Panaceum Group will be ready to interact with My Health Record from mid December 2018. Our patients will have the option to be able to load their key health information to their My Health Record with participating health practitioners.

Due to this update we have reviewed our practice privacy policy to recognise My Health Record, and how as a practice we maintain your privacy when interacting with My Health Record.

You will find our updated Privacy policy on our website. Please visit https://www.panaceum.com.au/privacy-policy/ to access it.

We would also like to use this opportunity to ask you to take a moment to think about your current consent for collection and use of your information. In particular, if there is anyone that you routinely want to be able to act on your behalf. For example pick up prescriptions, or check on your tests. If this is something you do want to have, please as our friendly staff for a new consent form next time you are in, and we will note down your preferences.

As always if you have any questions please speak to a team member or your GP.

Kind regards

Panaceum Group
